Portal Carers Wellbeing Break payment referral

Caring for someone can be hard work so taking time for yourself is important. The carer wellbeing break payment could help you do this. It is a one-off £300 payment to help you take a well-deserved break from your caring role whether that's to enjoy an activity, hobby or anything else that helps to give you a break away from your caring role. The carer wellbeing break payment is open to all adult carers of adults in Surrey who :

• are 18 years or older
• look after someone who lives in Surrey
• have not had a previous carer's payment, for example from a GP
• have not used a carer break offer
• are not getting support from us

The person the carer looks after must:

• be 18 years or older
• live in Surrey
• not have a social worker

You can apply as:

• the carer
• a friend or family member, with the carer's permission
• a professional

We want to make sure this is the right offer. We will need the carer's:

• contact details
• proof of address
• a form of id
• how taking a break will help

The more you tell us, the better. We may need to contact you to understand what support you need and if there are other ways to assist you with the full range of local community and carer support services in Surrey. The form should take around 10 minutes to complete. You will need to register with an email address. Our privacy notice explains how we store and use your information.
