Adult Professional Referral

This form is for professionals to refer an adult with care and support needs into adult social care. If you are a wishing to refer yourself please go to our adult self assessment instead.

This form needs to be completed from the perspective of the person with as much information as possible to enable the referral to be processed and triaged quickly.

Young people to the Transition Team. Surrey Children's professionals can use the Transition referral form.
An adult to the mental health social care teams. Please use the Mental health professional referral form.
