Carer Professional Referral

This form is for professionals to refer a carer into adult social care. If you are a carer referring yourself, please complete our carers self assessment instead.

• This form needs to be completed from the perspective of the carer, with as much information as possible to enable the referral to be processed and triaged quickly.

Please Note. It may take up to three working days to process this referral. Working days are Monday to Friday, 9am to 5pm. If the referral is urgent and requires an immediate response, please submit the referral form, make a note of the reference number, and call our Information and Advice Line on 0300 200 1005 to alert our staff to the urgency. Our out of hours emergency duty team can be contacted on 01483 517898. If the situation is life‑threatening, please contact the appropriate emergency service or mental health crisis line.
